We would like to establish an honest and open relationship with reference to payment for dental services rendered. We will make Individual arrangements for payment after your account has been established with us. If for any reason your agreed payment arrangement cannot be kept, notify us so that new arrangements can be made. Your consideration will greatly reduce our collection expense, thus helping us to maintain our fees at the most reasonable rates.
All accounts that remain unpaid for more than 45 days, without prior arrangements, will be submitted for collection. The same applies to any insurance accounts which has a balance after the insurance company has paid its portion.
Seldom is the case that dental benefits meet the expectations of the policyholder. We will make every effort to file insurance claims quickly to help you receive your full benefit. However, please understand that if your insurance does not pay you are responsible for paying the balance due.
Based on our years of experience we will provide you with the best treatment our professional ability has to offer. We feel that our treatment plans are reasonable and practical, and is in the best interest of you and your family's health. If you have any preferences, limitations or desires that might influence our suggested treatment, please discuss them frankly with us.
We divide our treatment into four (4) phases: 1. DIAGNOSIS - Finding out what is wrong or abnormal 2.CONSULTATION - Telling you what needs to be done 3.TREATMENT - Doing what needs to be done. 4.PREVENTION - Keeping it from happening again.
Dental cleanings vary depending upon the amount of debris present on the teeth and gums and the amount of bone loss that has occurred. Each patient will be evaluated during their exam, along with their exam the hygienist will determine their exact needs.
